By completing the new starter questionnaire, we can establish two things: whether a candidate is fit to perform a new role; and whether a work environment will have a detrimental effect on a candidate’s health.

The New Starter Questionnaire can be issued to new recruits or existing employees recruited into new roles. If you are the recruiting manager, you will need to go to the APOHS portal and click on the ‘New Starter Questionnaire’ page. Here, you will find the link and access code to send to the candidate or employee.
If you are the candidate or member of staff, you will be sent an invitation to complete the online questionnaire, with a link to the online form and an access code. If your organisation follows a different referral process (for instance, using a paper questionnaire), your recruiting manager will explain this.
